In-Person Service Request Form
Praise the Lord, Refuge Temple Ministries Family!
There are overwhelming requests to attend Sunday morning and Wednesday night services.
Our seating capacity has been reduced to maintain CDC recommended guidelines. Our concern is that families may drive to the church and find the seating capacity full and they have nowhere to sit.
The Service Request Form allows attendees to reserve seating for their families to prevent this disappointing inconvenience.
This reduction in seating capacity is temporary and normal service attendance will resume as soon as possible.
Fill out the Service Request Form to request seating for our Sunday or Wednesday Service.
A separate form must be completed for each service you wish to attend:
SUNDAY SERVICE REQUEST GUIDELINES:
1. Masks are required upon entering the church and must remain on during the entire service.
2. At this time, Sunday services are by 𝐈𝐧𝐯𝐢𝐭𝐚𝐭𝐢𝐨𝐧 𝐎𝐧𝐥𝐲.
3. You may 𝐎𝐍𝐋𝐘 attend the service that you were approved for and approvals 𝐃𝐎 𝐍𝐎𝐓 roll over to other dates.
4. The number of members permitted each Sunday will be according to CDC guidelines to accommodate adequate social distancing.
5. Temperature screenings will be taken at the door.
6. There shall be 𝐍𝐎 𝐂𝐎𝐍𝐆𝐑𝐄𝐆𝐀𝐓𝐈𝐍𝐆 among attendees.
7. Social distancing of 6 ft. must be maintained at all times while in the sanctuary.
8. 𝐃𝐎 𝐍𝐎𝐓 attend if you are sick, recently tested positive, or have been exposed to COVID-19 within the last 14 days.
9.Security will be at the door to ensure only those pre-approved will be allowed to enter the church. 𝗔𝗟𝗟 𝗢𝗧𝗛𝗘𝗥𝗦 𝗪𝗜𝗟𝗟 𝗕𝗘 𝗧𝗨𝗥𝗡𝗘𝗗 𝗔𝗪𝗔𝗬.
10. We want to ensure that the members attending service with you are household members only; we are not accepting guests at this time.
11. If you are chosen to attend the service, you will receive a notification by Saturday.
12. A new request must be submitted each week. 𝐑𝐞𝐪𝐮𝐞𝐬𝐭 𝐃𝐎 𝐍𝐎𝐓 𝐫𝐨𝐥𝐥 𝐨𝐯𝐞𝐫.
13. Please provide your own transportation to & from the church. There are no church transportation services.
14. Please refrain from contacting Sister Faunn for permission to attend the service.
